There is a new CEH Roster and CEH Request Form available. Please dispose of the old version and start using this new form.

CEH Request Form

Northeast EMS can no longer accept faxes or copies of CEH Rosters. We are now required to send the original CEH roster to Maine EMS to process. Maine EMS has been receiving information that is too hard for them to read and this makes the process of receiving credit for your CEH classes longer. We are sorry for any inconvience this may cause you. Thank you for your help and consideration in this matter.
